Health Questionnaire - Practice members fill out our personalized questionnaire on their current health state. These questions allow the doctors to determine what is going on within someone’s body in order to recommend proper action steps.
Diet Sheet & Journal - This is when our practice members record their dietary intake throughout a set amount of time.
